Keith Hill has been refereeing since he was fourteen, starting in the Hertfordshire Senior and Southern Leagues. He was promoted to the National List of Assistant referees in 1994 and to the National Referees List in 1998.

His first match as a League referee was a 2-1 win for Shrewsbury at home to Scunthorpe on 8 August 1998. Jamie Forrester gave the Irons a 1-0 lead but two goals in the last eight minutes gave Shrewsbury the three points. His career appointments include refereeing a semi-final of the FA Trophy, and being appointed as 4th Official on the 2002 Game of Unity in Kabul.
